This is a tiny category. The most-rated contact lens tracker in the US has 945 ratings after ten years on the store, and everything below it is in the double digits or lower. So "most popular" tells you very little here, and the retail apps that show up in the same search (1-800 Contacts, with over 140,000 ratings) are shops, not trackers.
The question that actually separates these apps is what happens on day 15, when you have forgotten the app exists. Three things matter: does the reminder repeat until you act, or fire once and vanish; can you see the countdown without opening the app (widget, Lock Screen, Watch); and does it treat your two eyes as two lenses, since they rarely start on the same day. A fourth thing almost nobody tracks: how the lenses feel. People do not quit contacts because they missed a date. They quit because of a slow slide into dryness and irritation that no calendar shows.

Most-used, no surprises: Contact Lenses Tracker by DOUBLE U. Free, nags until you replace, ten years of updates. Its Premium twin is the same thing for $29.99 up front.
Home Screen first: Contact Lens Reminder – Widget. It is the runaway leader in Japan (nearly 4,000 ratings) and its whole design is "never open the app": days remaining, replace-from-widget, a calendar widget.
Pay once, no subscription: Lensy ($1.99) or the DOUBLE U Premium app ($29.99).
Comfort matters as much as the date: LensLoop. A one-tap daily comfort check builds a trend so you notice the slide before it becomes a reason to give up on lenses. This is what I built it for, so weigh that accordingly.
